To access medication, a full diagnostic report is required for the young person. This report should be in line with the NICE guidelines. See terms and conditions for details of what is required to proceed to medication.

This questionnaire is to run through the health history of the young person to understand any risks the medication may pose. This must be completed with a GP.
Online video call appointment are used to discuss medication options, and start titrating (adjusting) the prescribed medication.
